Personal Care Aide
Overview
A Home Care Aide is needed to join our team. Following a one-on-one care plan in patients’ homes, you will be interacting directly with patients.
You work with a huge heart, not just giving individualized care and close attention to the people you serve, but also developing real connections with them. You are dedicated to fostering a healthy environment that satisfies patient demands, extremely organized, and detail-oriented.
To be successful in this position, you must possess excellent verbal and written communication abilities. Experience in hospice care or home health is a bonus.

Responsible For
- Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
- Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
- Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
- Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
- Give medications and immunizations
- Engage patients in exercises or other activities
