Overview

Alchemy’s Homecare Individualized Home Support (IHS) Program is designed to provide in-home assistance to people who live independently in their own homes.  People who are supported by the IHS program receive support based on their Level of Need.

How it Works

Individualized home supports cover community living services in four categories:

  • Community participation
  • Health, safety and wellness
  • Household management
  • Adaptive skills

When a person receives individualized home support without training, they must receive support in at least one of the community living service categories.
When a person receives individualized home support with training or individualized home support with family training, they must receive training in at least one of the community living service categories. They also may receive support in any of the community living service categories.

This category could include:

  • Public transit and pedestrian safety (e.g., safely getting in and around the community)
  • Use and accessibility of community resources
  • Public awareness and safety
  • Creation of an unofficial support system and networks
  • Abilities in interpersonal communication
  • Planning for amusement, socializing, and leisure
  • Creating skills for transportation requirements

This group could include:

  • Collaborating with the individual to schedule meetings, appointments, meaningful activities, and health care (e.g., physical, mental, and chemical).
  • Cueing, direction, supervision, instruction, or other support to carry out self-care (Note: See CBSM – Home care overview for more information. You cannot repeat the use of qualifying Medical Assistance [MA] state plan home care services.)
  • Support for health services, as outlined in Minnesota Statutes 245D.05
  • Assistance in developing and activating the person’s resiliency factors. (Example: activity management for overall health)
  • Support for the individual to develop and adhere to customized plans to achieve their health, safety, and well-being objectives.

This category may include:

  • Crisis prevention skills
  • Implementation of positive support strategies
  • Problem-solving
  • Sensory/motor development involved in acquiring functional skills
  • Support strategies for self-sufficiency
  • Support and training to increase positive behaviour resulting in the reduction or elimination of maladaptive behaviour
