What is 245D?

Certain Home and Community-Based Services (HCBS) provided to people with disabilities and those over the age of 65 are licensed by the Department of Human Services (DHS). The majority of the services are covered by one of Minnesota’s Medicaid waiver programs.

The Minnesota Statutes, Chapter 245D, and HCBS standards are part of a larger HCBS Waiver Provider Standards initiative to improve the dignity, health, and independence of those served.

Basic Support Services

These are services that provide the level of assistance, supervision, and care required to ensure a person’s health and safety. Each is linked to the section of the Community-Based Services Manual that contains operational information about that particular service. The Community-Based Services Program Manual contains information about services that require a license.

Individual community living supports (ILCS): For people who require reminders, cues, intermittent/moderate supervision, or physical assistance to live independently in their own homes.

24-hour emergency assistance: On-call counselling and problem-solving, as well as an immediate response to a health/personal emergency at a person’s home.

Companion services: Non-medical care, supervision, and socialization for people over the age of 18.

Homemaker services: Eligible services range from light housekeeping to housekeeping with incidental assistance with home management and/or daily living activities.

Night supervision: Provides overnight assistance and monitoring in the person’s home by an awake staff member.

Personal support: Services provided in a person’s home or community to assist them in reaching their full potential, increasing their independence, and meeting their community inclusion goals.

Respite care services: Short-term care provided in the absence or absence of the family member(s) or primary caregiver who would normally provide the care.
