Alchemy Home Healthcare is licensed under Minnesota Statutes, chapter 245D, to provide both basic and intensive home and community-based services.
What are 245D waiver services?
Chapter 245D is the Minnesota law that sets standards for home and community-based services (HCBS). These services are paid for through Medical Assistance waiver programs and help people with disabilities and older adults live safely and independently in their homes and communities, instead of in a facility.
Programs we accept

- Waiver services for HealthPartners and UCare members
Services are provided based on eligibility, approved service authorizations, and payer requirements.
