Everyday support that helps adults 65 and older stay safe, comfortable, and independent in their own homes.
What is ICLS?
Individual Community Living Supports (ICLS) is a service for older adults on the Elderly Waiver (and the Alternative Care program). It brings together reminders, cues, supervision, and hands-on help in one service, so a person can keep living in their own home instead of moving to a facility.
How our staff can help
ICLS covers six areas of support. Each person receives help in at least two of them, based on their plan:
- Active cognitive support: reminders and cues to help with routines and decisions
- Adaptive support: help adjusting to changes in abilities
- Daily living activities: help with tasks such as dressing, grooming, and eating
- Household management: meal preparation, laundry, light cleaning, grocery shopping, and paperwork
- Health, safety, and wellness: help keeping up with appointments and healthy routines
- Community living engagement: errands, outings, and staying connected to the community
Who it’s for
- Adults age 65 or older
- Enrolled in the Elderly Waiver (or Alternative Care), including HealthPartners and UCare members
- Who need reminders, cues, supervision, or physical help to stay in their own home
Good to know: Your case manager or care coordinator completes an ICLS Planning Form that tells us which areas of support you need. We follow that plan and keep your care team updated as needs change.
